Person Centred Tools
The use of Person Centred tools are at the cornerstone of how we deliver support as a service, prioritising a persons needs and preferences in their support plan. By identifying the person's unique strengths and abilities, these tools enable individuals to work collaboratively with their support team to develop strategies that promote greater autonomy, independence, and satisfaction in their daily life.
Below is a suite of tools that are available to anyone who wants them. They have a guide on how to use them and are beneficial to people who are looking to establish new supports and secure the ones that are working. Service providers are also encouraged to understand their use.
Person Centred tools have been around since the 1940’s and brought to prominence by Wolf Wolfensberger, a sociologist and researcher, in the 1970s.
At the bottom of this page there are some further links to resources that may help you on your person centred journey.
If you are a provider looking for training for staff in Person Centred Planning and the use of Person Centred tools, this is a service Stepping With Kindness is able to provide, please email any enquiries through to evan@steppingwithkindness.au
